general information and FAQs

Preventative Care Visits

For most insurance plans, preventative care is covered at 100%. Every adult has one annual or preventative visit per year.

Adult Preventive Exams include:

  • Diet and lifestyle assessment

  • Screening physical exam

  • Periodic pap smears, breast exams, prostate exam

  • Routine vaccinations

  • Lab tests per screening guidelines

Sometimes a problem or issue will be discussed during your preventative visit. In many cases, it can be easily addressed, but if additional tests, medications, and/or treatment recommendations are needed, the provider may add an office visit code onto your preventative visit and you will be responsible for a copay, co-insurance, or any amount that goes to your deductible.

We encourage patients not to save problems or health complaints for their annual or preventative visit, and instead, come into the office at the onset of any issue to address complaints as they arise. This allows us to give ample time to each issue or concern, instead of rushing through to cover many components in a short period of time.

Well-child Visits

For most insurance plans, preventative care is covered at 100%. Every child has 1-6 visits a year depending on their age.

Well-child visits include:

  • Assess growth and development

  • Breastfeeding support

  • Food introduction

  • Exercise recommendations

  • Routine vaccinations

  • Screening for medical and psychosocial problems

  • Anticipatory guidance (what to expect coming up)

Laboratory Tests and Medical Imaging

Although we may order these services, we do not generally bill for them with the exception of a small number of labs we run in the office.  Your bill for these services will come directly from whoever performed them and any questions you have about that bill should be addressed to that specific provider.

After Hours Care

A Ground Floor Health, a provider is available to take urgent after-hours calls on evenings and weekends. The pager number is on the general office voicemail. When you page the on-call provider, they will help you navigate the health concern in a timely manner, which may include deciding to go to urgent or emergent care, or to wait for the next day to schedule in the office.

Please be thoughtful with your calls and do not page for non-urgent issues, medication refills, lab orders or to schedule a visit. There is a $25 fee for after-hours pages. This service is not covered by health insurance plans.

Late Cancellation/No Show Visits

We require a 24-hour notice for cancelling or changing appointments.  If your appointment is on Monday, please cancel by the preceding Friday. Appointments that are canceled late or appointments that are missed or no-showed will incur a charge of $50 that is not covered by your health insurance plan.
