insurance coverage
The providers at Ground Floor Health are contracted with most insurance companies.
As a courtesy, we will bill visits to those companies we are considered in-network with. Keep in mind that we cannot guarantee coverage by your plan, and we recommend that you call your insurance plan before your appointment to be sure your visit will be covered. Scroll down to see the list of in-network insurance plans.
We do not bill for out-of-network visits, meaning if we do not have a contract with your insurance plan, you will be expected to pay for your visit. We will provide documentation that you can submit to your insurance company so you can request reimbursement from your plan.
For those who do not have insurance or who have out-of-network insurance, we offer a Time of Service discount of 15% if you pay the visit in full the day of your visit, otherwise, the total amount (not discounted) will be billed monthly.
